Unkown Medal from HMS Hampshire

I have what i believe is some type of service medal from the HMS Hampshire. I found it in my grandmothers jewelry box. I do not know if it is from her side of the family which is from Norway or my Grandfathers who has a family lineage of Naval Military service. My grandfathers family however has been in America for two centuries.
